Open Enrollment

December 1^st through 15^th is open enrollment. Please complete an enrollment form, and return to the Human Resources Office no later than December 15^th by 5:00 p.m. to make changes, add and/or remove dependents to the health plan.

If you would like to add, make changes, or remove any of the following Colonial Supplemental Insurance, you can contact Wayne Smith at 1-800-720-7998.

· Short-term Disability Insurance

· Accidental Death and Dismemberment Insurance

· Cancer 1000 Insurance

· Critical Illness Insurance

If you would like to add, make changes, or remove any of the following Symetra Supplemental Insurance, please the complete and turn in the Symetra Supplemental form, by December 15^th to the Human Resources Office, 1900 Mariposa Mall, Ste. 121.
