| One of the enhancements to our Health and Welfare Program this year was the addition of a weight management program. This is a 12 month program designed to help those with a BMI>30 without and >27 with co-morbid diseases lose weight. To qualify for this program, participants must be referred by their primary care physician and receive approval through our third party administrator, PCMI. This program is currently being offered by Dr. Swartz through the Advanced Bariatric Center. For more information, please visit the website at http://www.advancedbariatriccenter.net/ .
At the completion of the 12 month program if satisfactory weight-loss is not achieved and the participant has a BMI>40 without and >35 with co-morbid disease, the participant may then be considered eligible for appropriate weight loss procedure(s) as approved by AMA (American Medical Association) guidelines . For more information on coverage, please see our Employee Health Plan Summary Plan Document, page 14 (click here to go to the SPD) or contact the Human Resources Office.
