| Description |
The Babies First Outreach team is responsible
for outreaching in the contracted census tracts within
the community to assist pregnant women and women with
children access medical care. |

Assist pregnant women find a medical doctor
for prenatal care and help get Medi-Cal or other ways
to pay for medical care. Case monitoring is provided to
ensure clients are receiving continuous prenatal care.
Bus tokens are provided to clients experiencing difficulty
getting to medical appointments. |

all pregnant women receive adequate and continuous prenatal
care to reduce infant mortality, and that their infants
and children have access to quality primary preventive
health care. |
